The form that follows will allow you to share a memory about how a mentoring relationship has positively impacted your life; this memory may be posted on www.FFNC.org. Foster Friends of North Carolina will publish a collection of mentoring memories. Should your entry be chosen we will be in direct contact. Foster Friends of NC reserves the right to edit and to use, re-use, reproduce, publish, re-publish and display your story for advertising or other promotional purposes to promote our mission. Foster Friends of NC also has the right to reject submitted pieces. We ask that you provide your email address so that we can contact you should your narrative be chosen for other pieces (your email address will not be shared or sold to any other organization). Last names will be omitted to protect privacy. All submissions become the property of Foster Friends of North Carolina.

Those that are 13 years of age or younger must have a parent or legal guardian's written permission to submit an entry. A staff member from Foster Friends of NC will be in contact to receive the required permission before the youth's entry is posted on www.ffnc.org. You may also download the Parental Permission form and send it to Foster Friends of North Carolina.
